A Clinical Study on 284 Cases of Primary Carcinoma of the Liver

Abstract
|
|
|
A clinical study was performed on 284 cases of histopathologically proven primary carcinoma of the liver encountered at Presbyterian Medical Center at Jeonju during period of 10 years and 8 months and the results obtained were as followings:
1) The incidence of primary carcinoma of the liver among total other malignant tumors during same period was 4.3% and among admitted patients was 0.53%.
2) The ratio of male to female was 5.2 : 1. The age ranged from first decade to eighth decade. The highest in male was 4th decade and in female 6th decade. Pathological classification of the hepatoma was 242 cases of liver cell carcinoma 18 cases-of poorly differentiated carcinoma, 16 cases of cholangiocarcinoma, 6 cases of hepatoblastoma and case of mixed cell carcinoma.
3) The chief complaints were epigastric fullness and discomfort; indigestion, weight loss and anorexia in order of frequency.
4) Frequent misdiagnosis were liver cirrhosis, liver abscess and stomach carcinoma.
5) Incidental findings of other diseases were pulmonary tuberculosis, peptic ulcer and liver cirrhosis, etc.
6) Incidence of remote metastasis were 20% of total cases, the most frequent being in the lung.
7) Hepatomegaly was the most constant finding and was seen 97% of cases while splenomegaly was 4.5%.
8) There were no specific findings in laboratory data indicating primary liver cell carcinoma but elevation of alkaline phosphatase and SGOT were 73% and 70%.
9) Clonorchis Sinensis infestation wss found in 24.6% of cases.
10) Liver scanning showed cold area of 70%.
11) Average duration from onset diagnosis was 60 days.
12) The average surival time after 1iver biopsy was 85.8 days and the longest onw was 3 years.
13) We performed hepatic radiation in 6 cases, surgical lobectomy in 4 cases and 5-FU administration in 3 cases but effect was not good.
